It really depends on the current and the drag the fly creates moving through the water column.

Q: Do you just stick the regular loop through and peg it?

I only peg the larger ones and that depends on where it sits on the leader.

Q: If you want to add to the line and already have flies tied on, do you need to cut them off and string this on first and re-tie?

You have to put the strike indicator on first then your nymph. Use the jam stop to adjust your depth. The only reason you would need to take it off is if you would want to switch to something like a dry fly.
